Registration Information

Enrollment is based on a first-come, first-serve basis from the date all of the necessary paperwork is received by the Enrollment Coordinator.

Please see the Ready-Set-Go page if your child is of kindergarten age, but will not be attending kindergarten.

Every new child must go through a screening. To make an appointment, please call 349-1629.

When coming in for the screening, the following paper work is required:

• Certified copy of birth certificate*
• Income documentation*
• Shot records*
• Physical* or appointment card
• Dental* or appointment card
• Registration fee (See fee scale)
• Custody Papers (If applicable)
• Registration Packet 12-13 for Southwest (Click on link to print out packet)
• Registration Packet 12-13 for Union St. (Click on link to print out packet)

* The state of Ohio mandates all children must have Hematocrit & Lead testing done by your phycisian
